Webpages launched to support people in Powys affected by cancer

People affected by cancer in Powys can now access new web pages for the Improving the Cancer Journey Programme, bringing together clear information on local support.

People affected by cancer in Powys can now access new web pages for the Improving the Cancer Journey Programme, bringing together clear information on local support.

The new online site is designed for adults living with or affected by cancer across Powys — whether newly diagnosed, undergoing treatment, recovering, adjusting to life beyond cancer, living with a palliative cancer diagnosis, bereaved or supporting a family member or friend.

The webpages feature a link to the Cancer Community Connector for Powys, allowing people to self-refer or refer someone else. Through this service, people can access a supportive “what matters” conversation and receive personalised signposting to local services to support a range of everyday concerns, including:

  • Health and wellbeing
  • Returning to work
  • Counselling and emotional support
  • Transport
  • Money worries
  • Planning support, such as making a will or managing pet care

The site also highlights ways for people to:

  • share their cancer experiences to help improve services
  • get involved in the Journeying Together Group, which brings people with lived experience together to help shape the programme
  • learn more about Getting ready for their Cancer Treatment, supporting people to improve their physical and emotional wellbeing before cancer treatment begins.

Macmillan Lead Nurse for Cancer and Palliative Care in Powys, Louise Hymers explains: “Improving the Cancer Journey in Powys is about listening to what matters most to people affected by cancer and making sure they are connected to the right support, at the right time.

“This new webpage makes it easier for people and their families to find help locally and to know they don’t have to navigate their cancer journey alone.”

Gall pobl sydd wedi’u heffeithio gan ganser ym Mhowys nawr gael mynediad at dudalennau gwe newydd ar gyfer y Rhaglen Gwella’r Daith Canser, sy’n dwyn ynghyd wybodaeth glir am gymorth lleol.

Mae’r wefan ar-lein newydd wedi’i chynllunio ar gyfer oedolion sy’n byw gyda chanser neu yr effeithir arnynt gan ganser ledled Powys – boed wedi cael diagnosis newydd, yn cael triniaeth, yn gwella, yn addasu i fywyd y tu hwnt i ganser, yn byw gyda diagnosis o ganser lliniarol, wedi galaru neu’n cefnogi aelod o’r teulu neu ffrind.

Mae’r tudalennau gwe yn cynnwys dolen i Gysylltydd Cymunedol Canser Powys, sy’n caniatáu i bobl hunanatgyfeirio neu atgyfeirio rhywun arall. Drwy’r gwasanaeth hwn, gall pobl gael mynediad at sgwrs gefnogol “beth sy’n bwysig” a derbyn cyfeiriadau personol at wasanaethau lleol i gefnogi amrywiaeth o bryderon bob dydd, gan gynnwys:

  • Iechyd a lles
  • Dychwelyd i’r gwaith
  • Cwnsela a chefnogaeth emosiynol
  • Trafnidiaeth
  • Pryderon ariannol
  • Cymorth cynllunio, fel gwneud ewyllys neu reoli gofal anifeiliaid anwes

Mae’r wefan hefyd yn tynnu sylw at ffyrdd i bobl:

  • rhannu eu profiadau o ganser i helpu gwella gwasanaethau
  • cymryd rhan yn y Grŵp Teithio Gyda’n Gilydd, sy’n dod â phobl â phrofiad bywyd ynghyd i helpu llunio’r rhaglen
  • dysgu mwy am baratoi ar gyfer eu triniaeth canser, gan gefnogi pobl i wella eu lles corfforol ac emosiynol cyn i driniaeth canser ddechrau.

Mae Prif Nyrs Macmillan ar gyfer Canser a Gofal Lliniarol ym Mhowys, Louise Hymers, yn esbonio: “Mae gwella’r Daith Canser ym Mhowys yn ymwneud â gwrando ar yr hyn sydd bwysicaf i bobl sydd wedi’u heffeithio gan ganser a’u cysylltu â’r gefnogaeth gywir, ar yr amser cywir.”

“Mae’r dudalen we newydd hon yn ei gwneud hi’n haws i bobl a’u teuluoedd ddod o hyd i gymorth yn lleol a gwybod nad oes rhaid iddyn nhw lywio eu taith canser ar eu pennau eu hunain.”
